It is usually counted from the first day of the last menstrual period, often shortened to LMP. When menstrual dates are uncertain, pregnancy dating may instead be established by a health professional using the available clinical information.
Gestational age is expressed in completed weeks and additional days. For example, a gestational age of 8 weeks and 3 days means eight full weeks and another three days have elapsed from the pregnancy's recognised starting point.

What Is Fetal Age?
Fetal age estimates the amount of time that has passed since conception or fertilisation. In this article, fetal age and conceptional age refer to this approximate time since conception.

Why Is Gestational Age Usually Two Weeks Greater?

In a typical cycle, conception may occur around two weeks after the last menstrual period began. Those first approximately two weeks are included in gestational age even though conception has not yet occurred.
This is why a pregnancy can be described as four weeks' gestation when the approximate time since conception may be closer to two weeks.
This system is a standard dating convention. It does not mean conception or pregnancy biologically began on the first day of the last period.
Gestational Age and Fetal Age Example
Consider a pregnancy with a gestational age of 8 weeks.
The eight-week gestational age includes the time counted from the last menstrual period before conception is assumed to have occurred. In a typical cycle, the approximate time since conception may therefore be around six weeks.
Ovulation and conception may have occurred earlier or later, so this comparison remains an estimate.
